PreDisc was distinguished for its innovative approach to preventing dependency and improving the quality of life of older people.<\/p>\n<p>PreDisc, co-financed by ERDF funds through the Interreg Sudoe programme, aims to prevent disability and promote active and healthy ageing by encouraging regular physical activity during and after hospitalisation. Its model is based on the individualized prescription of physical exercise programs during hospital admission, with continuity after discharge thanks to follow-up from Primary Health Care, also supported by an innovative digital eHealth tool.<\/p>\n<p><img decoding=\"async\" class=\"size-medium wp-image-59576 alignleft\" src=\"https:\/\/interreg-sudoe.eu\/wp-content\/uploads\/2026\/01\/premiados-300x145.jpg\" alt=\"\" width=\"300\" height=\"145\" \/>This approach represents a change in the traditional model of care, moving from a vision focused solely on illness to one focused on functionality, well-being and autonomy for older people.
