Il est exp\u00e9riment\u00e9 dans 3 contextes sociosanitaires (valeur cible 3) : 6 centres de soins de sant\u00e9 primaires, 4 centres de services sociaux \/ living labs et 1 clinique priv\u00e9e sp\u00e9cialis\u00e9e.
