Europe is moving towards an ageing society, with the SUDOE territory, and specifically rural environments, having a higher percentage of elderly people and depopulation. Ageing brings with it the appearance of frailty and disability, associated with increased use of health resources and hospital admissions.
The PreDisc project aims to prevent disability in older people by promoting regular physical activity during and after periods of hospitalization. This is achieved by prescribing a personalized program during hospitalization and its continuation after discharge, thanks to its follow-up by Primary Care and facilitated the development of an innovative eHealth tool. The project also includes a multicenter clinical trial to generate scientific evidence and promote the integration of these programs into routine clinical practice.
In addition, PreDisc focuses on promoting an active ageing model, especially in rural areas, through the development of technological tools, training of health personnel, home visits and events in rural communities, The aim is to reduce unnecessary hospital admissions. The aim is to promote equal access to health by reaching rural areas and limiting the need for older people to move.
