The PreDisc project seeks to prevent disability in older people through the prescription of physical exercise during hospitalization and its continuity after discharge. Its impact is expected in hospitals, primary care and rural areas of the Sudoe space. In its first year, PreDisc has made progress on implementation despite some administrative challenges. Specifically, the strategy for the new healthcare model has been defined, and key steps have been taken in launching the clinical trial and developing the eHealth tool.
From hospital to rural areas
The multicenter clinical trial, which will assess the impact of exercise on the functional recovery of hospitalized patients, has already started in Navarra (HUN). The centres in Portugal and Andorra are expected to be added in the coming months. This essay aims to evaluate the impact of a physical training program on the functional recovery of hospitalized patients with the objective of preventing hospital disability.
Another milestone is the creation of an eHealth tool, designed to improve physician-patient communication and personalize post-hospital exercise patterns. The first prototype is expected to be available by mid-2025, when its test phase will begin.
Training and dissemination
To ensure the success of the PreDisc model, training for health professionals is being designed and will begin this year. The project has also taken significant steps in its dissemination strategy. A communication plan has been designed to ensure that the project is disseminated among health professionals, authorities and citizens. In March, the launch event was held in Pamplona, which was attended by the health adviser of the Government of Navarra and all partners. Awareness events have also been held in Andorra, Portugal and Navarra (Roncal Valley), aimed at both health professionals and older people.
Perspectives for 2025
2025 will be key to consolidating the progress achieved. The next steps include the multicenter clinical trial, testing of the eHealth tool and training of healthcare workers. In addition, community events to promote active ageing will be intensified, reinforcing the impact of PreDisc in the participating regions.
