This pilot action, coordinated by FSBE, aims to validate the effectiveness and feasibility of the SHAPE transfer model across diverse hospital contexts (public, private, rural, or urban). Concurrently, it will assess the model’s capacity for large-scale dissemination.
The pilot seeks to test the transfer model with a view to its widespread adoption within the network of Orientation Partners (OPs):
SACYL (Spain): The Ministry of Health of Castile and Leon, representing a network of 14 public hospitals and 247 health centers in rural areas.
FJD (Spain): A private hospital integrated into the QuirónSalud network, which comprises 50 hospitals and 100 health centers.
ISJD-Lisbon (Portugal): A social and hospital institution with a dense network of community partners.
The main objective is to demonstrate that the SHAPE model can be adapted to different environments (public, private, rural, or urban), ensuring its relevance, scalability, and sustainability.
The three OPs will conduct implementation and transferability tests, targeting 50 patients per partner, for a total of 150 participants. These tests will include:
Local adaptation of the SHAPE pathway, considering the specific characteristics of each territory (infrastructure, population, local needs).
Integration of adapted digital tools, such as pre-configured tablets, to facilitate program access for patients with limited digital access.
Mobilization of their respective networks to test a broader expansion of the project within the 900 establishments they have access to.
This pilot action will validate a methodology for expanding SHAPE’s reach, facilitating its adoption by other territories and institutions beyond the initial partners. It will help identify necessary adjustments and continuously enrich the project’s deliverables.
