SHAPE Action Plan for Large-Scale Adoption and Sustainability
This action plan, jointly developed by SHAPE partners under the coordination of FSBE, aims to ensure the large-scale adoption and lasting sustainability of the model. Based on the results of pilot actions and transferability tests, it organizes the necessary actions to integrate SHAPE into institutional and hospital practices.

Institutional Impact: The white paper, coordinated by FSBE with the support of SACYL, will compile the results of the RCTs, best practices, and adoption strategies. Aimed at policymakers, it will be promoted in regional seminars and institutional networks to encourage SHAPE’s integration into regional, national, and European health systems.

Transferable Model: The transfer model, developed in WG3, is a guide for adapting SHAPE to different hospital environments. It will be applied during tests with the three Orientation Partners (OPs).

Practical Tools: The Implementation Kit (IK), designed by FSBE, includes guides, training content, and digital tools for patients and professionals. Available online, it facilitates SHAPE’s adoption in hospitals by reducing operational barriers.

Sustainable Economic Model: The business plan will propose stable funding sources such as integration into health insurance systems, alliances with private actors (Nutricia Medical, IMA), or partial patient contribution.

Professional Commitment: Ensured through Online Training (OT) and the Virtual Community of Practice (VCP), this will foster continuous learning that guarantees uniform and sustainable adoption of the model.
