The global SHAPE pathway proposes a joint and structured strategy, led by all partners, to address a common problem: the lack of a system for preventing fragility acquired during hospital stays. The strategy aims to create a patient journey that allows for the early diagnosis and treatment of fragility, ensuring continuity between the hospital and the return home. Developed across four convergent axes, this collective strategy offers an integrated and operational approach that responds to the challenges of active and healthy aging.
Strengthening Prevention Capacity: The strategy is based on the standardization and dissemination of methods for detecting fragility, integrating validated diagnostic tools (SPPB, MUST, etc.).
Progressive and Personalized Treatment (Stepped Care Model): This model adjusts the intensity of care according to the evolving needs of patients, through progressive and adapted attention. This approach fosters patient adherence and optimizes available resources.
Accessibility for All: The strategy includes care facilitated by a centralized web platform that offers a single point of access for patients and healthcare professionals. Additionally, pre-configured tablets will be provided for patients distant from the digital environment or healthcare systems, ensuring the inclusion of rural and marginalized populations.
Strengthening Professional Commitment and Their Role as Prescribers: The strategy seeks to increase the competencies of healthcare professionals and Specialized Sports Educators (SSEs) in prescribing physical activity.
Each beneficiary participates in the definition and execution of the strategy, playing a key and complementary role in this shared strategy that they will adopt to address the identified problem.
