Inaugural session of the SAM project (Smart Digital Twin for Social Prescription)

📍 On 7 October 2025, we held the Inaugural Technical Health Meeting, which marked the official launch of the project, bringing together a total of 25 partners from Spain, France and Portugal.

⭐ The session opened with a clear message: to move towards a more preventive and community-based approach, supported by health assets and the Social Determinants of Health, connected through an intelligent digital tool.

After a connection activity using an interactive map, the afternoon’s work focused on two main milestones:

✅ Validation of a common matrix to assess health assets, agreeing on shared criteria — relevance, accessibility, evidence, governance/replicability — and identifying a key challenge: how to also measure “intangible” assets — organisational or governance-related — without adding complexity to their use in the field.

✅ Review of the SDoH model to develop a clearer and more adaptable version, incorporating improvements in terminology, weighting according to context, the role of structural determinants, and considerations regarding data quality and comparability.

🔚🤝 We closed the day with aligned ideas, a shared language and clear next steps: turning today’s work into practical tools for implementation in the territory.
