SHAPE Project: Pilot Action for Fragility Treatment
Within the framework of the SHAPE project, the main pilot action aims to demonstrate the feasibility and clinical effectiveness of the model for treating fragility acquired during hospitalization. This action is based on the implementation of Randomized Controlled Trials (RCTs) involving five Hospital Research Partners (HRPs).
The RCTs will scientifically demonstrate the benefits of an innovative pathway that combines personalized care, a structured, staged journey that optimizes resources according to needs, and digital tools through the SHAPE platform. The model’s impact will be evaluated within a framework study protocol using key indicators such as physical function (SPPB), quality of life (EuroQol), and nutrition (MUST).
The SHAPE model is characterized by the innovative integration of digital tools that facilitate access to the care offered, a pathway that allows for early diagnosis and treatment of fragility, and progressive care focused on the real needs of patients.
During the recruitment phase, 500 fragile patients will be included, distributed equally among the HRPs (100 per HRP). Care will extend over a 4-month period and will be based on the Stepped Care approach, which ensures progression adapted to the specific needs of patients. At each stage, evaluations will be carried out to adjust the intensity or mode of care, moving progressively from collective and digital monitoring to more individualized and intensive support, if necessary.
The key indicators will be evaluated at 4 time points: hospital admission and discharge, as well as at the beginning and end of the care program. This rigorous evaluation will generate robust clinical data that will validate the model’s effectiveness and establish a scientific basis for its large-scale adoption in new health institutions.
