Social prescribing, an internationally recognized approach, involves recommending local assets in healthcare settings to improve population health and well-being. The World Health Organization (WHO) published a Guide for implementing social prescribing in primary care in 2022, yet there’s a gap between health strategies (which highlight health assets and social determinants) and practice, with limited experience in Spain and Portugal, and none in France. The common Sudoe challenge is to implement social prescribing in primary care to enhance population health and well-being. The intelligent digital twin SAM (acronym for SAlud, Social, Activos y Mapa – Health, Social, Assets, and Map) is a web service that allows healthcare professionals to visualize social determinants and health assets near the patient, customizing healthy lifestyle routines and guidelines in real time, adapted to the local territory. This socio-health innovation project valorizes territorial heritage and local services that represent health assets, generating added-value opportunities, fostering community health, and signposting assets in the territory. The pilot is deployed across the 3 countries, anchored in 8 focal points of different problems across 8 NUTS II regions with 2 types of implementation: “local primary care” in 6 focal points and “special” in 2 (one cross-border ES-PT and another for transfer to specialized oncological medicine in FR). SAM operates in rural and urban territories, seeking territorial balance and sociodemographic cohesion, targeting populations aged 6-18 and over 60. Implementation follows the 7 phases established by WHO, with SAM being innovative in health asset mapping (phase 4) and objective outcome evaluation (phase 7) – for the first time transnationally. Findings will be transferred to regional and national health policies, promoting cross-border cooperation, the valorization of local heritage for health, and equitable access to assets. A long-term strategic planning plan for SAM will guide its adaptation to opportunities, resilience to challenges, and transfer to other countries.
